Approval matrices age. Managers change, temporary cover becomes permanent, and card limits rise without a matching update to the expense policy. Claims then clear on informal authority that exists only in someone’s calendar invite.

Watch for these signals: claims above the stated threshold with a single approver; bulk approvals on Fridays before close; and new joiners receiving different verbal rules from different line managers.
